You should also watch for frayed cables, or cables where the coloured wiring inside is exposed. This needs to be delt with immediatly, either by an elecrician, or by being thrown away.

There are several things that can cause this. Carrying an item by its cord, this is a bad idea. Never unplug an item by pulling on its cord. You should also never leave electrical items near water, or in the bathroom.

Don't run cables under carpets, this may stop them from being a trip hazzard, but they may wear through without being noticed.
